NHCH is comprised of two large clinics, Makalapa at Joint Base Pearl Harbor-Hickam and Kaneohe Bay at Marine Corps Base Hawaii. Other smaller clinics are located at Camp H.M. Smith; Naval Communications and Naval Computer and Telecommunications Area Master Station, Pacific, Wahiawa and Pacific Missile Range Facility, Barking Sands, Kaua'i. Welcome Center
Your introduction to care at NHCH begins at the Welcome Center, where our PRIME beneficiaries begin a one-stop check-in process to enroll to NHCH. The staff will assist you with TRICARE enrollment, Medical Record updating, Primary Care Manager (PCM) assignment and initial appointments for any care. Active duty members will also update their Periodic Health Assessment (PHA) requirements, part of the Individual Medical Readiness (IMR) process. TriWest representatives and Health Benefits Advisors are available to answer TRICARE and enrollment questions.

During the check-in process, information about several resources, including TRICARE Online (http://www.tricareonline.com, a website that allows you to make medical appointments), Exceptional Family Member Program and Advance Directives will be provided. Please bring ID cards, Social Security numbers (for all enrolling), private health insurance cards, medical records, dental records (if active duty) if in your possession and immunization records when you check-in. We will request that your medical record (civilian or military) be sent from your last provider. A Request for Medical/Dental Records, DDForm 877, will facilitate this process if completed prior to moving to Hawaii. This form is available from your current military treatment facility (MTF) Medical Records Department. If we do not have your record, you can still complete the check-in process. If you have never had a military healthcare system medical record, we will make one for you. A copy of your civilian records will also be requested.

Exceptional Family Member Program (EFMP): The Exceptional Family Member Program (EFMP) identifies and documents the special medical, mental health and/or educational needs of family members to ensure that those needs are considered during the assignment process. Enrollment in EFMP is mandatory for all family members of active duty service members with needs requiring specialized services. TRICARE Dental Program: Information on the TRICARE Dental Program may be obtained directly from United Concordia Companies, Inc. by visiting their Website http://www.tricaredentalprogram.com or by calling (800) 866-8499. The TRICARE Family Member Dental Plan is a dental insurance plan for family members of active duty military to receive dental care with civilian dentists. All family member dental care is provided within the civilian community. To ensure your family members' dental needs are met, we encourage active duty military to enroll them into the TRICARE Family Member Dental Plan (TFMDP). This will avoid costly dental bills. For more information, contact your Health Benefits Advisor Office, or the United Concordia office at http://www.ucci.com or call (800) 866-8499. Military retirees may call (888) 838-8737 to enroll in TRICARE Retiree Dental Plan or view/enroll at http://www.trdp.org.

After-hours Provider Advice Line: If you need to speak with a medical provider anytime the clinics are closed, simply call (808) 473-0247, ext. 3 and you will be connected to an answering service who will contact the on-call provider. The provider will be able to help you decide how to treat an illness after hours or whether you should go to the Emergency Room. The provider has no access to labs, X-rays, appointments or medical records. 

Urgent Care Walk-in Clinic at Makalapa Clinic (JBPHH): Offers NHCH TRICARE PRIME beneficiaries the convenience of being treated for an acute, non-emergent medical illness or injury during the times listed below without having to call for an appointment. 

Weekend Urgent Care Walk-in Clinic hours are: 
Saturday, 8 to 11 a.m. and 1 to 3 p.m. 
Sundays and holidays, 8 to 11 a.m. 

What's Urgent Care? Urgent care services are medically necessary services which are required for an illness or injury that would not result in further disability or death if not treated immediately, but require professional attention and have the potential to develop such a threat if treatment is delayed longer than 24 hours. An urgent care condition could be a sprain, sore throat or rising temperature. Urgent care services are provided by Physicians, Physician Assistants and/or Nurse Practitioners. 

Emergency Services: Dial 911 or report to Tripler Army Medical Center Emergency Room. For life threatening emergencies, report to nearest Emergency Department. 

What's an Emergency? TRICARE defines an emergency as a medical, maternity or psychiatric condition that would lead a "prudent lay person" to believe that a serious medical condition exists, or that the absence of immediate medical attention would result in a threat to life, limb or sight, and requires immediate medical treatment or which has painful symptoms requiring immediate attention to relieve suffering. For emergencies, such as profuse bleeding, unconsciousness, chest pain and obviously broken bones, dial 911, go to Tripler Army Medical Center or the nearest emergency department.
